Note:

• Pending violations or convictions not yet reported by the court may affect your eligibility
• If there are requirements you must complete before you are eligible to reinstate, you will be given that information and may need to go in-person to the DMV to complete the transaction
• You may need to file proof of insurance (SR-22)
• You may need an Ignition Interlock Device (IID)

Have these ready:

• Your Wisconsin Driver License or ID card number
• The last four digits of your Social Security number
• Your date of birth

Online process notes:

• Once the online transaction is completed you cannot cancel the transaction or receive a refund.
• If a new driver license is being issued, you will also be issued a driving receipt. Print this receipt. It will serve as your driver license until you receive the card in the mail.
• If your Social Security number is not entered, you will only be able to check your eligibility and may not complete your reinstatement online.
• This online system is for driving privileges. To obtain ID card reinstatement requirements, contact us by email.
• After you've checked your eligibility, completed the requirements, and paid your reinstatement fee, you can confirm that your Wisconsin driving privilege is valid by checking your driving privilege status online.

To reinstate in-person:

Complete the application online and make an appointment for your visit.

If you visit a DMV customer service center without an appointment, you will need to complete a Wisconsin Driver License Application MV3001 to reinstate.

Please plan to arrive at least 90 minutes before the center is scheduled to close. You will be required to pay the reinstatement fee.
