Improve the health and well-being of women and families in Wisconsin by purchasing Wisconsin Women's Health Foundation license plates.

Your $25 donation will be re-invested in communities all over Wisconsin:

  • 50% will support Wisconsin Women’s Health Foundation outreach and education programs that reach thousands of Wisconsin women and families each year.
  • 50% will support Community Health Grants, which provide funding for community-based women’s health programs in Wisconsin.

The donation may be in honor or in memory of someone. You will receive additional information with your Wisconsin Women’s Health Foundation license plates. Things you should know:

When you request Wisconsin Women’s Health Foundation license plates, you pay:

  • $25 Wisconsin Women’s Health Foundation donation
  • $15 issuance fee or an annual $15 personalized plate fee, if requested
  • The regular registration fee for your vehicle if your current plates expire within three months or you have no plates
  • At each plate renewal, you will pay the $25 donation and the regular registration fee for non-personalized Wisconsin Women’s Health Foundation.

How to apply

DMV Customer Service Centers do not issue Special or Personalized License Plates you must mail in your application.

To apply for Wisconsin Women’s Health Foundation license plates by mail, please send the following items:

  1. If th​​​​e vehicle you want to register with Wisconsin Women’s Health Foundation ​Wisconsin Women’s Health Foundation plates is already titled/registered in your name, send:
    • ​​A copy of your Certificate of Vehicle Registration or show the complete vehicle description on the Wisconsin Women's Health Foundation License Plate Application MV2957
    • $25 Wisconsin Women’s Health Foundation ​donation
    • $15 issuance fee or $15 personalized plate fee
    • The annual registration fee if your current plates expire within the next three months ​or you have no plates
  2. If the vehicle is not titled/registered in your name, send:
    • A completed Wisconsin Title & License Plates Application MV1, or MV11 for dealer sales, including all other fees due
    • A completed Wisconsin Women's Health Foundation License Plate Application MV2957
    • The Certificate of Title assigned to you ​by the seller
    • $25 Wisconsin Women’s Health Foundation donation
    • $15 issuance fee or $15 personalized plate fee
    • The annual registration fee if you do not have plates or your current plates expire within the next three months
  3. Make the check or money order made payable to: Registration Fee Trust​.

Personalized license plates

Personalizing your Wisconsin Women’s Health Foundation license plates is optional. A $15 personalized plate fee is required each year in addition to the $25 donation and regular annual registration fee. ​On the Wisconsin Women's Health Foundation License Plate Application MV2957​, check which option you would like us to follow if your request is not available. Please allow an additional 4-6 weeks for delivery.
