What is a driver license or instruction permit (temps):
- Can be used for driving
- Can be used as photo identification
- Can be used for voting
Use the online process to:
- Replace a driver license (including CDL) or instruction permit that has been lost or stolen
- This online service will order a duplicate driver license or duplicate instruction permit
- Verify or change your residence and/or mailing address
- CDL holders are required to get a replacement driver license within 10 days of their address change
- Have your license or permit mailed to your temporary out-of-state address if you are a Wisconsin resident temporarily living out of Wisconsin
- Surrender your driver license (no longer driving) and replace it with a Wisconsin ID card (use ID card online service)
Note: If you are under the age of 18, adult
sponsorship will be required (provide sponsor's driver license or ID card number). Proof of driver education course completion (submitted electronically by your school) must also be on file with the DMV.
You MUST
apply in person if:
- Your driver license or instruction permit (temps) is expired or will be expiring within 30 days (but you may be able to
renew online)
- You changed your
- eyeglass or contact lens restriction
- personal identifier (e.g., weight or hair color)
- name must show
proof of name change
- gender (must show proof by court order or physician’s note)
- status of an
invisible disability
- picture on your driver license or instruction permit
- You are applying for an original (first time)
REAL-ID compliant driver license
- You are not a U.S. citizen and/or do not have a
verifiable Social Security number on record with DMV
- You are under the age of 18 and proof of driver education course completion has not been electronically submitted to DMV yet
- You have applied for four duplicates online within the last year
Have these ready
The online application will ask you for the following information:
- Valid debit or credit card
- Your Wisconsin Driver License or ID Card number
- The last 4 digits of your Social Security number
- Your date of birth
- Your zip code
OR
- Valid debit or credit card
- Your first name, middle initial, and last name
- Your complete Social Security number
- Your date of birth
- Your zip code
AND
- A printer with standard white 8.5 x 11-inch paper to print your temporary paper driver license or instruction permit (temps)
- A valid email address
- Valid debit or credit card
What happens next?

Receipt: A receipt, including your photo, will be emailed to you upon successful completion of this online transaction. This receipt is acceptable photo identification for voting and serves as your driver license until your card arrives in the mail.

Timing. Your driver license will be processed and mailed to you through the USPS using first class mail. You will receive it within 10 business days.

Once the online transaction is completed, you cannot cancel it or receive a refund. Verify that you have entered the information correctly before completing payment information.
Note: This online service is not available from 2-3 a.m. each day due to system updates.
This service may not be available on Sundays from 6–9 a.m. due to regular system maintenance.

online service and submit it electronically, or print and bring with you. Depending on your eligibility, you may also be able to schedule an appointment.

- If you choose not to use the online application, download and print the Wisconsin Driver License Application (form
MV3001) and bring it with you.
When you visit the DMV service center
If you are under 18 years of age and applying for a duplicate instruction permit, you must also provide:
- A new MV3001 signed by the Behind-the-Wheel instructor and the Sponsor.
- The Sponsorship section on the MV3001 must be signed by a parent or legal guardian “And” must be notarized or witnessed by DMV employee.
If you are under 18 years of age and applying for a duplicate driver license you must also provide:
- The Sponsorship section on the MV3001 must be signed by a parent or legal guardian “And” must be notarized or witnessed by DMV employee.
Apply by mail
- Note: Wisconsin residents can only apply by mail for a duplicate driver license when the Wisconsin resident is currently out of state.
- U.S. citizens may apply for a duplicate (replacement) driver license one time by mail. Future applications will require you to visit a DMV to have an updated photo taken.
