Becoming an organ and tissue donor

You can help save and improve lives by registering to be an organ and tissue donor when you renew your driver license or ID card. People of all ages can donate regardless of health history.

Your gift will make a difference through transplantation, therapy, research or education. If you are 18 years old or older, your decision may not be overridden by your family or any other person, so it's important to share your decision with your family.

  • Complete a Wisconsin Driver License Application MV3001 or Wisconsin Identification Card (ID) Application MV3004
  • Check the box indicating you wish to register as a donor

Registering means that you authorize the donation of your organs, tissues and eyes upon your death. You can also register or remove your name from the registry at DonateLifeWisconsin.org.

The Wisconsin donor registry is maintained by the Wisconsin Department of Health Services.

$2 Contribution to Donate Life - Wisconsin

You now have the option to contribute $2 to Donate Life - Wisconsin when you apply for your next driver license. You will see the $2 donation itemized on your receipt.

  • Check the box indicating you would like to add $2 for organ, tissue and eye donation education efforts
