Upload your medical certificate (Fed Med card) - Tier 1 drivers
- Tier 1 drivers are required to submit their Fed Med card to the DMV and keep it updated. Once you have uploaded your Fed Med card you will be able to print a confirmation of successful upload.
Note: The confirmation is not valid to present to law enforcement or as proof you meet the commercial medical standards. The actual medical certificate and information on your record are the true verification that you meet these standards.
- Drivers who have certified to operate at Tier 2, 3 or 4 are not required to upload their Fed Med card. However, you may still be required to have a Fed Med card due to state statutes or employer policy. You do not have to furnish a copy of the card to the DMV, unless you are a Tier 3 driver applying for a license renewal or replacement at a DMV Customer Service Center.

- If you have already certified Tier 1, 2, 3 or 4 you may view and print your information from this website.
- If you would like to change your tier, you will need to visit a
DMV Customer Service Center. The following are required:
- A completed Wisconsin Driver License Application
MV3001. We recommend that you complete all forms online, print and take them with you to the DMV Customer Service Center.
- A completed Commercial Driver Certification: Tier of Operation

MV3230 defines each of the tier levels. When you visit a DMV Customer Service Center to apply for your CDL, you will be asked to make your selection.

You'll need:
- Your Wisconsin driver license number
- The last four digits of your Social Security number
- The product number from your driver license (11 digit number from the back of your license)
- If you are uploading a new Fed Med card, you will need it available in an acceptable file type not to exceed 5 MB. File types accepted are PDF, JPG, GIF or PNG.
You will not be able to use this system if:
- You do not have a Wisconsin commercial driver license (CDL)
- You are under the age of 18
Processing time
Although most certificates are reviewed and added to the driver record in two to three business days, it may take up to 10 business days to complete this process.
Reminders for CDL operators
Keep your current physical address on file with the DMV. Failure to do so can result in a delay when receiving correspondence from the DMV. If you need to update your address, please visit your local
DMV service center to apply for a duplicate driver license.
