These instructions are for WisDOT contract specialists and local program management consultants. The list sets forth the number of signed copies of the contract and supporting documents that must be sent to central office.
The contract submittal checklist and DT25 should be completed by WisDOT or local program management consultants only.
Consultants should submit two signed copies of each contract and supporting documents to WisDOT. In the case of three party contracts, the consultant should submit an additional signed copy of the contract and supporting documents for each local unit of government involved.
Contracts less than $3,000
- 2 copies of the contract and required supporting documents with original signatures of the consultant and Region/Bureau Director or designee.
Contracts of $3,000 or more
- 3 copies of the contract and required supporting documents, all with original signatures of the consultant and a signature block for the WisDOT Contract Manager.
- For 3-party contracts, add an additional copy for each local unit of government involved.
- Form DT25 Recommendation to Governor
Work Orders less than $50,000
- 2 copies of the work order and required supporting documents with original signatures of the consultant and Region/Bureau Director or designee.
Work Orders of $50,000 or more
- 3 copies of the work order and required supporting documents, all with original signatures of the consultant and a signature block for the WisDOT Contract Manager.
Amendments less than $50,000
- 2 copies of the amendment and required supporting documents with original signatures of the consultant and Region/Bureau Director or designee.
Amendments of $50,000 or more
- 3 copies of the amendment and required supporting documents, all with original signatures of the consultant and a signature block for the WisDOT Contract Manager.
- For three party contracts, add an additional copy for each local unit of government involved.
